Here the example which you have given is output of combination of images, means there are two images for this ,

one-  Arrow Image
two- Rectangle box.

YOu need to show these images when textbox is empty.

It isn't impossible to scavenge the internet and find the source code for a similar styled tooltip. However, if you see the source of this html from where you posted the screenshot, you would get it easier. 

And whatever be the styles, I would recommend jQuery tooltip plugin to show this tooltip. Irregular shapes, with formatted text are easily done thru jQuery plugins.
